ACKNOWLEDGMENT OF RISK AND RELEASE
PARENT/GUARDIAN of MINOR
I, ___________________________ (full name), understand that participation in this Admissions event and Sewanee Outing Program trip is voluntary. I realize that the activities described expose my child to risks not present in everyday life and which could result in personal injury, illness, death or damage to personal property. Some of these risks include, but are not limited to, the following:
hiking on rugged and sometimes unstable terrain
falling and other dangers associated with climbing, scrambling and caving
injuries from insects, animals, or plants
forces of nature including lightning, flood, cold, and heat
remoteness of location, such as in a cave, where medical services may be less accessible than usually expected
physical exertion associated with outdoor activity (heat, dehydration, sun exposure, etc.)
falling rocks
dangers inherent to a construction site
dangers associated with swimming and boating in lakes
travel in a vehicle not driven by me
I understand that by reading and signing this acknowledgment my child assumes all responsibility for his/her actions. My child agrees to abide by the rules and regulations set forth by the Sewanee Outing Program, the program's director, and the University of the South. My child also understands that, at any time, he/she can choose not to participate in an activity if he/she deems the activity to be beyond a personal level of comfort and safety.
In consideration for the University sponsoring this program, I agree that I will not hold the University, its officers, agents, or employees liable for any personal injury or property damage my child might sustain while participating in the Admissions event and Sewanee Outing Program trip, and I hereby release the University, its officers, agents, and employees from all liability arising from or related to my child’s participation in the Admissions event and Sewanee Outing Program trip.
